Turkesterone has no published side-effect profile in humans. The only human study on turkesterone itself followed 11 men for 24 hours. So the honest answer for a US buyer is that the documented risks in a turkesterone product almost always belong to the other ingredients stacked beside it: ashwagandha, tongkat ali, piperine and colostrum.

That is a different question from the one most pages answer, and it changes what you should do before your first capsule. You are not really evaluating turkesterone. You are evaluating a four- or five-ingredient formula, and only the label tells you which one you are about to swallow.

The human evidence on turkesterone is 24 hours long

One acute study is the entire human record for turkesterone. Published in Muscles in 2024, it gave turkesterone to 11 men and measured them before dosing, at three hours and at 24 hours (Grabowska et al., 2024). It found no significant effect on IGF-1, resting metabolic rate, or lipid and carbohydrate metabolism, and the authors reported that circulation and digestion were tolerated well across that window.

Twenty-four hours cannot show a liver signal, a hormonal signal, or anything that needs weeks to appear. A supplement people take daily for months has been observed in humans for one day.

The 10-week trial everyone cites as "the turkesterone study" is not one. It tested ecdysterone, a different phytoecdysteroid, in 46 young men doing strength training (Isenmann et al., 2019, Archives of Toxicology). That trial reported gains in muscle mass and bench-press one-rep max, and — the part relevant here — no increase in markers of liver or kidney toxicity. It is a genuinely reassuring result for ecdysterone. "No reported side effects" describes missing evidence, not safety

Nobody has reported side effects from turkesterone because almost nobody has looked. There is no post-marketing surveillance system that collects turkesterone complaints, no multi-week human safety trial, and no adverse-event catalogue to draw a number from. Absence of reports is a statement about how little research exists.

Compare that with an ingredient that has been watched. Ashwagandha has case series, a national registry entry and a scoping review of its liver injuries — because clinicians documented them. The difference is not that ashwagandha is more dangerous than turkesterone. It is that ashwagandha has been observed long enough for anything to show up.

Treat "no known side effects" the way a scientist would: an open question, not a clean bill of health.

Ashwagandha: the liver signal that is real but uncommon

Ashwagandha carries the best-documented adverse effect in this formula. A 2020 report in Liver International described five cases of ashwagandha-associated liver injury from Iceland and the US DILIN network (Björnsson et al.). The pattern is consistent: onset two to twelve weeks after starting, a cholestatic or mixed picture, jaundice and itching. No patient developed liver failure, and liver tests returned to normal within one to five months in four of the five.

A 2026 scoping review pulled together 13 publications covering 25 patients and found the same shape — injury after weeks of use, cholestatic or mixed predominating (Cureus). The largest single series, from India, is reported in Hepatology Communications (2023).

Two things put this in proportion. Twenty-five documented patients is a small number against how much ashwagandha the world takes, so the risk is uncommon. And the ashwagandha trial most often cited for stress used 300 mg twice daily for 60 days (Chandrasekhar et al., 2012) — twice the daily amount on this label. Dark urine, yellowing eyes or skin, persistent nausea or itching after a few weeks on any ashwagandha product is a reason to stop and call a clinician, not to wait it out.

Tongkat ali: an unsettled regulatory picture

Tongkat ali is where the evidence genuinely disagrees with itself, and no US label will tell you that.

In 2021 the EFSA panel assessed tongkat ali root extract as a novel food in the EU at up to 200 mg per day and concluded that safety was not established (EFSA Journal). The reason was genotoxicity: a positive in-vitro chromosome-aberration test, plus a positive in-vivo comet assay at 2,000 mg/kg body weight in first-contact tissues.

The counterweight is a 13-week subchronic study of tongkat ali root powder in rodents that found no adverse effects up to 2 g/kg/day, and proposed an acceptable daily intake of up to 1.2 g for an adult (eCAM, 2013). Both are real findings. A US supplement facts panel simply is not required to mention that a European regulator looked at this ingredient and declined to sign it off.

Piperine: the ingredient that changes your other medicines

BioPerine is black pepper extract, added at 10 mg to help absorption. That is exactly why it deserves a second look if you take prescription medication.

Piperine inhibits human P-glycoprotein and CYP3A4 — two of the main systems your body uses to clear drugs (Bhardwaj et al., 2002, JPET). In that work it inhibited digoxin and cyclosporine transport with IC50 values of 15.5 and 74.1 µM. Slowing those systems can raise the blood level of a medicine you are already taking, at a dose your prescriber set assuming nothing was interfering.

The practical version: if you take anything with a narrow safety margin — a blood thinner, an anti-rejection drug, a heart-rhythm drug, an anti-seizure drug — the piperine in a turkesterone stack is the ingredient to raise with your pharmacist. It is also the cheapest question you will ever ask, because a pharmacist will check it in under a minute.

Colostrum: a dairy allergen hiding in a sports supplement

Bovine colostrum is milk. That is the whole point of it, and it is also the risk: a person with a milk-protein allergy can react to a capsule nobody described to them as a dairy product. Nothing about the words "turkesterone" or "advanced formula" signals dairy, and this listing does not flag colostrum as a milk allergen anywhere. If you get drug-tested, the side effect is a doping-control one

For a tested athlete the relevant risk is not physiological. Ecdysterone was added to the WADA Monitoring Program, as noted in a 2021 study that gave 12 volunteers a single 50 mg dose and tracked urinary elimination (Metabolites). Monitoring is not the same as a ban — WADA monitors substances to see how widely they are used before deciding anything — but it means the compound class is on the radar and it is being measured in urine.
