What NMN Safety Trials Report So Far

Most NMN safety data comes from small, short human trials. None of them found serious problems. None ran long enough to tell us about years of use. Here is what each trial measured.

The shortest test used single doses. Ten healthy men took one oral dose of 100, 250 or 500 mg NMN. Over the next 5 hours, researchers saw no significant clinical symptoms and no changes in heart rate, blood pressure or oxygen saturation (Irie 2020).

Longer studies came next. Healthy older men took 250 mg NMN a day for 6 or 12 weeks. It was well tolerated, with no significant harmful effect, and blood NAD+ rose significantly (Igarashi 2022). An 8-week open-label study gave 11 healthy middle-aged Japanese men two 125 mg capsules once daily before breakfast. It was well tolerated, and NAD+ in blood mononuclear cells increased (Yamaguchi 2024).

The largest dose-range trial enrolled 80 healthy middle-aged adults. They took placebo or 300, 600 or 900 mg NMN once daily for 60 days. Researchers found no safety issues, and blood NAD rose in all NMN groups (Yi 2023). The highest daily dose came from a trial of 31 healthy adults aged 20 to 65. They took 1,250 mg NMN once daily for up to 4 weeks. Measurements did not change beyond normal physiological variation, and there were no severe adverse events (Fukamizu 2022).

The limits matter as much as the results. The longest trial here lasted 12 weeks. Group sizes ranged from 10 to 80 people. Industry co-sponsored the Yi trial. The NMN supplier funded the Fukamizu trial, and its authors worked for that supplier. "No severe adverse events" in a few dozen people over a few weeks is reassuring. What the Betaine (TMG) Research Shows

First, why some formulas pair TMG with NMN. The enzyme NNMT uses SAM as a methyl donor to turn nicotinamide into methylnicotinamide. This step makes SAH, a precursor of homocysteine (Li 2024). Betaine (TMG) is a methyl donor in the methionine cycle, mainly in the liver and kidneys (Craig 2004). It gives methyl groups to homocysteine to form methionine (Zeisel 2003).

That is a mechanistic rationale, not a proven clinical benefit. No published human trial has tested NMN + TMG against NMN alone. We explain the reasoning in more depth in NMN and TMG together: why the pairing matters.

Now the side-effect question. High-dose betaine has one clear signal: cholesterol. A meta-analysis of 6 randomized trials using at least 4 g a day for 6 to 24 weeks found that total cholesterol rose by 0.34 mmol/L. It found no significant effect on LDL, HDL or triglycerides (Zawieja 2021). A larger 2022 meta-analysis found that betaine raised total cholesterol by 14.12 mg/dL and LDL by 10.26 mg/dL. It lowered homocysteine by 1.30 µmol/L. It had no effect on triglycerides, HDL, fasting glucose, CRP, liver enzymes or blood pressure (Ashtary-Larky 2022).

Dose context is the key point. People usually get about 0.5 to 2 g of betaine a day from food (Olthof 2003). Wheat germ, wheat bran and spinach are among the richest sources (Zeisel 2003). The 250 mg of TMG in our label serving is below even that usual dietary intake. It is far below the 1.5 to 6 g a day tested for homocysteine and the 4 g or more linked to cholesterol changes. No trial tested 250 mg. So you should not expect homocysteine lowering from this amount. You also cannot assume the cholesterol finding applies at this amount.
