NMN and NR both raise NAD+ in humans, and both have been well tolerated in trials. The difference is what each one has been shown to move after that. The authors framed their physiological findings as initial insight pointing toward future trials, not as a settled effect.

A third trial gave 12 aged men 1 g of NR daily for 21 days. It raised the skeletal-muscle NAD+ metabolome and produced anti-inflammatory transcriptomic signatures — but it did not deliver the improvement in muscle mitochondrial bioenergetics the researchers set out to find (Elhassan 2019, Cell Reports).

The fourth is the one that changed how careful readers weigh NR. Forty obese, insulin-resistant men aged 40–70 took 1,000 mg of NR twice daily — 2,000 mg a day — for 12 weeks. Insulin sensitivity, endogenous glucose production, glucose disposal and glucose oxidation were not improved. Neither was resting energy expenditure, lipolysis, lipid oxidation, or body composition. The trial's own conclusion is that 2,000 mg/day of NR appears safe but does not improve insulin sensitivity or whole-body glucose metabolism in this group (Dollerup 2018, American Journal of Clinical Nutrition).

What the NMN Trials Found

The NMN evidence is younger and smaller, and it reaches further into outcomes.

A 10-week randomised, placebo-controlled, double-blind trial gave 250 mg of NMN daily to postmenopausal women with prediabetes who were overweight or obese. Insulin-stimulated glucose disposal, measured with a hyperinsulinaemic-euglycaemic clamp, increased after NMN and did not change on placebo. Skeletal-muscle insulin signalling increased alongside it (Yoshino 2021, Science). The clamp is the reference method for measuring insulin sensitivity, which is what makes this result hard to wave away.

A second trial ran a dose ladder. Eighty healthy middle-aged adults took placebo, 300 mg, 600 mg or 900 mg of NMN once daily for 60 days. Blood NAD+ rose significantly in every NMN group at day 30 and day 60, highest at 600 mg and 900 mg. Six-minute walking distance improved significantly against placebo in all three dose groups at both time points, with the longest distances at 600 mg and 900 mg. The Dose Gap Most NMN vs NR Comparisons Skip

Read the table's two metabolic trials next to each other. The NR trial that found nothing on insulin sensitivity used 2,000 mg a day for 12 weeks. The NMN trial that did find something used 250 mg a day for 10 weeks — one eighth of the dose, for a shorter time.
