That step produces SAH, a precursor of homocysteine (Li 2024). The idea is that NMN leads to NAM, that clearing NAM draws on methyl groups, and that TMG can help supply them.

Some points matter for beginners:

  • Blood NAD went up. In Yi 2023, blood NAD rose significantly in all NMN groups at day 30 and day 60. Industry co-sponsored that trial.
  • Some physical tests moved, some did not. Six-minute walking distance improved vs placebo in Yi 2023. In Liao 2021, VO2max itself did not change. In Igarashi 2022, the gait speed and grip results were only nominally significant, and the authors said they "should be validated in larger studies."
  • Some metabolic markers did not change. HOMA-IR, a measure of insulin resistance, did not differ from placebo in Yi 2023. Yoshino 2021 did find higher muscle insulin sensitivity, but only in one specific group of women.
  • Short-term safety looked acceptable. Single doses of 100, 250 and 500 mg caused no significant clinical symptoms or changes in heart rate, blood pressure or oxygen saturation over 5 hours in 10 healthy men (Irie 2020). The NMN supplier funded the 1,250 mg Fukamizu trial and employed its authors.

What the research did not measure: the pairing. Every trial above tested NMN. None tested NMN + TMG against NMN alone.

3. The Label Dose vs the Doses in Studies

TMG: 250 mg. This is where beginners should adjust their expectations. Usual dietary betaine intake is roughly 0.5 to 2 g per day. In healthy adults, 1.5, 3 and 6 g per day for 6 weeks lowered fasting homocysteine by 12%, 15% and 20% vs placebo (Olthof 2003). At 4 g per day or more, a meta-analysis found homocysteine fell by 1.23 µmol/L (McRae 2013).

Those high doses also had a trade-off. At 4 g per day or more, total cholesterol rose by 0.34 mmol/L (Zawieja 2021). A larger 2022 meta-analysis found higher total cholesterol (+14.12 mg/dL) and LDL (+10.26 mg/dL), lower homocysteine (-1.30 µmol/L), and no effect on triglycerides, HDL, fasting glucose, CRP, liver enzymes or blood pressure (Ashtary-Larky 2022).

The 250 mg of TMG in this product is far below every one of those amounts. It is below even usual dietary intake. Do not expect homocysteine lowering from 250 mg. Also, do not assume the cholesterol finding applies at 250 mg. No trial tested 250 mg.
