What the 2021 Runner Trial Found

The best-known NMN exercise study is a 2021 randomized, double-blind trial in the Journal of the International Society of Sports Nutrition. It enrolled 48 recreationally trained runners, 40 men and 8 women aged 27 to 50. Each group took 300, 600 or 1,200 mg of NMN a day, or a placebo, for 6 weeks.

Everyone kept training. Sessions lasted 40 to 60 minutes, 5 to 6 times a week. After 6 weeks, the 600 mg and 1,200 mg groups showed bigger gains than placebo in oxygen uptake and in power at the first and second ventilatory thresholds. Those thresholds roughly mark the pace where breathing gets harder.

The same paper also reports what did not move. VO2max, O2-pulse and peak power did not change from baseline in any group. So the result is a better use of oxygen at sub-maximal effort, not a higher ceiling.

Two limits matter. The trial was small and ran for only 6 weeks. And every participant was already training hard, so it says nothing about NMN without exercise.

What Trials in Older Adults Show

Two other trials looked at physical function rather than athletic performance.

A 2022 trial in npj Aging gave healthy men aged 65 and older 250 mg of NMN a day, or placebo, for 12 weeks. NMN was well tolerated and raised NAD+ in whole blood. The researchers saw nominal improvements in gait speed and left-hand grip strength. Nominal means the signal was small and needs larger studies. NMN did not change muscle mass or body composition.

A 2023 trial in GeroScience gave 80 healthy middle-aged adults placebo or 300, 600 or 900 mg of NMN once a day for 60 days. Blood NAD rose in every NMN group. Six-minute walking distance increased more than placebo in all three NMN groups, with no safety issues found. This was not an exercise-training study, so it cannot say how NMN and a workout plan interact.

A 2026 network meta-analysis of 27 studies and 1,599 people sums up the pattern. NMN showed a dose-dependent improvement in endurance, while neuromuscular strength stayed the same. No NAD+ precursor improved perceived fatigue.

No published human trial has tested NMN before a workout against NMN after a workout. Any article that tells you one window is better is guessing.
